Class Absence and Medical Excuses
The Student Health Center will not give written excuses for individual missed classes. However, if you are sick and cannot go to class, schedule an appointment at the Student Health Center, where our health care provider will examine you and offer you a treatment plan. You may also request a "Verification of Health Center Visit" form.
This form will contain your name, BU student ID number, date and time of your visit, your signature and the signature of the provider. You may show this form to your prof(s) as evidence of your visit.
You cannot obtain a Verification of Health Center Visit form if you don't come to the health center on the day(s) you miss class(es). If you miss class because you are sick, be sure to contact all of your professors as soon as possible, so missed class work can be made up or obtained from others.
If your illness requires you must miss three or more days of class, the health center will send a "Notification of Extended Illness" to all of your professors, which will verify your absence is due to medical reasons.
If your illness occurs while you are away from school, the health care provider caring for you must send verification of your illness to the Student Health Center before the "Notification of Extended Illness" will be sent to your professors.
The health center will not give out any information regarding your medical situation to anyone: family, professors, friends, or even other health care facilities, without written consent. Professors may not call the health center to obtain any information on your medical absence. (See HIPAA)
