Depo-Provera Guidelines for Receiving Injection

Must provide the following prior to scheduling the Depo-Provera shot: (May fax to (570) 389-3417 Student Health Center)

1. A yearly order from prescribing provider, including:

a. Name of medication
b. Dosage
c. Route
d. Administration frequency
e. Date last dose given

2. Record of a GYN exam or Pap Smear within past three years

First dose must be given by the prescribing provider. The order must be reviewed and approved by our Medical Director prior to administering the dose. Student must bring the medication to the Health Center appointment.
