Forms to Complete for Briefing Meeting

  • Position Advertisement  (Samples provided by email once briefing is scheduled)
  • Position Advertisement Request (S Drive Template)
  • Criteria Rating Sheet (Samples provided by email once briefing is scheduled)
  • Interview Questions (Samples provided by email once briefing is scheduled)
  • Teaching Demonstration Form (Samples provided by email once briefing is scheduled) if applicable

Forms to Complete for Mid-Search Meeting

  • Documentation of Search: at mid-search (S Drive)
  • Documentation of Screen: at mid-search (S Drive)
  • Search & Screen Committee Minutes available to date

Forms for Appointment Packet

  • Position Control Form, complete with appropriate signatures
  • Official transcripts for all coursework
  • Application for Employment for Faculty, Management and Administrators (available online and S Drive) to be completed by candidate
  • Resume/Vita
  • Statement that a Background Check has been processed and complete (Notification and Authorization to Conduct Employment Background Investigation (available online and S Drive) to be completed by candidate)
  • Payroll Statement and Reference Authorization Form (available online and S Drive) to be completed by candidate
  • Reference Checklist/Employment Verification (S Drive) to be completed by Search Committee
  • Letters of Recommendation (if required by Dean/Supervisor)
  • Documentation of Search: at appointment (S Drive)
  • Documentation of Screen: at appointment (S Drive)
  • Minutes/Memos stating approval of the candidate

Copies of “Diversifying the Faculty – A Guidebook for Search Committees”, published by the Association of American Colleges and Universities, are available to all departments and search chairpersons in the Office of Social Equity, Waller Administration Building #114.
